Service

Forever30 ("Service") is a membership program operated by Pure Life Now LLC. Membership provides access to clinical consultations, protocol design, and member pricing for medications dispensed by an independent licensed pharmacy.

Eligibility

You must be at least 18 years old. Medication prescriptions and dispensing are subject to clinical review by a licensed clinician and applicable state and federal law.

Not medical advice

Content on this site is informational only and does not constitute medical advice. The clinician–patient relationship is established only after intake review and acceptance.

Billing & cancellation

Membership is $25/month, billed in advance. The first month is waived for new patients. Cancel any time through the billing portal — your access continues until the end of the current billing period. Medication purchases are billed separately.

Refunds

Membership fees paid after the first waived month are non-refundable except where required by law. Medication orders are non-refundable once dispensed by the pharmacy.

Acceptable use

You agree to provide accurate intake information, follow clinician guidance, and not resell or redistribute medications.

Limitation of liability

To the fullest extent permitted by law, Pure Life Now LLC's total liability is limited to amounts you have paid us in the previous 12 months.
