COACHING SERVICES AGREEMENT, PAYMENT & MEDICAL DISCLAIMER
This wellness program includes coaching and support services only. Medical evaluation and treatment are provided exclusively by independent licensed medical providers. Any medication costs are passed through at provider pricing for convenience and are not marked up or sold by Pure Life Now. Pure Life Now does not practice medicine, provide medical care, or profit from medical services or medication.
1. Nature of Services
I understand that PURE LIFE NOW LLC provides wellness and lifestyle coaching, which may include fitness training, nutrition guidance, accountability support, and other non-medical services. These services are not medical advice, diagnosis, or treatment.
2. Independent Medical Provider
As part of this program, I may be referred to or connected with an independent, licensed medical provider (the “Provider”). The Provider — not PURE LIFE NOW LLC — is solely responsible for evaluating my health, determining whether any medication is clinically appropriate, prescribing if indicated, and coordinating with a licensed pharmacy for dispensing. Any prescribed medication will be shipped directly from a licensed pharmacy to me. PURE LIFE NOW LLC is not a pharmacy and does not sell, dispense, or control prescription drugs.
3. Program Fee & Medication Coverage
- I am enrolling in a wellness and lifestyle coaching program provided by PURE LIFE NOW LLC.
- My payment to PURE LIFE NOW LLC is only for coaching, accountability, wellness support, and non-medical services.
- I understand PURE LIFE NOW LLC does not provide medical care, prescribe medication, or make medical decisions.
- My medical evaluation, treatment decisions, and any prescriptions are solely between me and the independent licensed medical provider.
- I understand medical providers and pharmacies operate independently from PURE LIFE NOW LLC.
- Any medical treatment costs (including consultations, labs, medications, or supplies) are separate from my program fee.
- For convenience, PURE LIFE NOW LLC may facilitate payment processing to the medical provider or pharmacy, but does not mark up, profit from, or provide medical services or medication.
- Whether medication is prescribed is the sole discretion of the licensed provider after medical evaluation.
4. Liability Waiver
I acknowledge that participation in any wellness or fitness program carries inherent risks. I release and hold harmless PURE LIFE NOW LLC, its owners, staff, and representatives from any claims or liability arising from my participation in this program. I further agree that PURE LIFE NOW LLC is not responsible for the medical care or outcomes provided by the independent Provider.
5. Health Responsibility & Indemnification
I understand it is my responsibility to discuss any health concerns, conditions, or treatments (including compounded medications) with my primary care provider or other qualified medical professional. Certain treatments offered by the independent Provider may not be FDA-approved but may be lawfully prescribed for individual use. I agree to indemnify and hold harmless PURE LIFE NOW LLC and its representatives from claims or liabilities related to my participation in wellness services or to medical care received from the independent Provider.
6. Corporate Practice of Medicine (CPOM) Disclaimer
I acknowledge and agree that:
- PURE LIFE NOW LLC is not a medical practice and does not employ or control licensed medical providers.
- All medical care, including any examination, diagnosis, prescribing, or dispensing of medications, is provided solely by an independent licensed Provider.
- PURE LIFE NOW LLC does not bill for or sell medical services or prescription medications.
- I authorize PURE LIFE NOW LLC to transmit my payment for medical services and/or prescriptions directly to the Provider or pharmacy at the price determined by that Provider or pharmacy, solely as an administrative convenience, and without markup or profit.
- My medical relationship is exclusively with the independent Provider, and any prescribed medication is dispensed by a licensed pharmacy in accordance with state and federal law.
- This program is subject to all applicable state corporate practice of medicine and fee-splitting restrictions.
7. Acknowledgment of Terms
By signing below, I confirm that I have read and understood this agreement, and I consent to the liability waiver, payment terms, and CPOM acknowledgment above.
(I understand that PURE LIFE NOW LLC does not recommend, influence, or control medical treatment decisions, and my participation in coaching is not contingent on receiving medication.)
